Direct Deposite Authorization Form
POLICY AND PROCEDURE
ALL Employees at Companion Home Care LLC must elect to have their paycheck deposited directly into their bank account every pay period.
To start direct deposit, this document must be read and signed along with the direct deposit authorization form. If you have any questions, please ask prior to signing this form.
You are responsible for verifying that the amount deposited is accurate for each weekly payroll period. It is also your responsibility to verify that the funds have been deposited in your account before you attempt to withdraw it or write checks, making personal transactions against it. Companion Home Care LLC. is not responsible for banks errors, whereas your banking institution may make errors or fail to make direct deposits in a timely manner. Companion Home Care LLC. is not responsible for overdrafts or bounced checks or any fees, charges or other problems that arise because you failed to verify the direct deposit. If you discover an error in the direct deposit, contact your bank and try to resolve the problem. If your bank cannot resolve the problem, notify Companion Home Care LLC for assistance.
Participation in direct deposit will require you to provide Companion Home Care LLC with information about the bank account and bank routing number where you want your money deposited. A separate form will be provided for you. It is your responsibility to notify Companion Home Care LLC, in writing if you close your direct deposit account or your account number changes. It is your responsibility to notify Companion Home Care LLC in writing if anything else occurs with your account that could disrupt direct deposit. Please contact Companion Home Care LLC if there are any changes in bank account.
Each bank has a different policy about when it will post deposits and how they make deposited funds available to you. These differences in policies may affect your direct deposit. Holidays may also cause delays in your ability to access the money deposited. Companion Home Care LLC is not responsible for your bank’s policies, practices, acts, or failure to act.
By electing to use direct deposit and signing below, you acknowledge that direct deposit uses the electronic transmission of funds, and that problems can sometimes occur with electronic transmission of funds at the Companion Home Care LLC’s bank, at your bank, or elsewhere which could delay in the transfer of your net weekly pay. To minimize delay in transmission of payroll, Employee must verify the accuracy of your clock in and clock out. If you did not rectify clock in and out issues at the end of the pay period, Employee will have to wait until the following pay period to receive your direct deposit.
Direct Deposit is a required form of payment for Employees at Companion Home Care LLC.
Employees are responsible for replacement of any lost ATM and bank card.
